If you are submitting a Personal Information Request Form on your own behalf or behalf of your child, please select the applicable option(s) below, and then provide the requested information to identify yourself. We may require further information to verify your identity.

Part B

If you are submitting a Personal Information Request Form as an authorized agent of a U.S. resident, please check below what requests you are making and the name and email address of the person on whose behalf you are submitting the request (and the name of that person’s child, if applicable).

At the time of submitting the request, please also send us a signed authorization from the person on whose behalf you are submitting the Personal Information Request Form, which clearly authorizes you to make the request, or proof that the person has given you power of attorney pursuant to applicable state law to our email address: vtechkids@vtechkids.com.

For requests to delete, requests to know, and requests to correct, please instruct the person for whom you are acting as agent to verify the person’s own identity directly with us. The person may submit such verification and confirmation by calling our toll-free number at 1-800-521-2010 or contacting us by using "Contact Us” on our website. This verification/confirmation requirement will not apply if you obtained from the person, and submitted to us, proof of power of attorney pursuant to applicable state law.
